On-Line Forms Here you will find a collection of all the online forms that are currently available on the RMIN secure intranet website. Basic Criminal Intelligence Analysis Training Form -Designed to provide RMIN members with a foundation in the analytical tools used to conduct criminal intelligence analysis. It is designed for local, state, and federal law enforcement officers and analysts who are involved in complex investigations. This course is AZ P.O.S.T. certified for 24.0 hours of continuing education. On-Line Analytical Casework Request Form - Include a brief description of the criminal activity and the names of other law enforcement agencies involved in the case.Fill out the necessary fields on the form and then press submit. The form is electronically sent to the appropriate individuals. On-Line Inquiry Request Form (PDF) - Use this form to request background searches for any criminal information on subjects, organizations, or businesses suspected of being involved in felony multi-jurisdictional criminal activity. On-Line Training Registration Form - Use this form to register for co-sponsored training from RMIN On-Line Feedback Form - In order to provide better service to our members RMIN has developed an on-line feedback page. Our members can place suggestions, problems, praise, etc about our services, website, conferences and more. Please take the time to let us know how we are serving you or can serve you in the future. Membership Renewal Form (PDF) - Current member agencies that have had a change in agency head must submit a Membership Renewal Form, signed by the agency head. Membership Application Form (PDF) - Agencies requesting initial/first-time RMIN membership must submit the Membership Application Form signed by the agency head. Individual Membership Application Form (PDF) - Officers requesting RMIN membership must submit the Individual Membership Form signed by the agency RMIN representative. Co-sponsored Training Request (Sample Letter) Individual Training Request (Sample Letter) Investigative/Confidential Funds Request Form (PDF) - This form does require a signature; therefore it can be filled in online but then it needs to be printed and signed before it is mailed. Please mail this form to: RMIN, Inc Financial Forms RMIN Vendor Contact Information and Remittance Address - To be accompanied with the IRS W-9. Used for Training Requests and Investigative Confidential Funds Request. Filling Out Online Forms - Printing Once you have clicked on the on-line form you wish to submit, you will notice some fields are in bold lettering. These fields are required fields and will not allow you to submit the information until the required field is filled in. Once the information has been submitted you will then receive a confirmation that your information was received. You can then print that page for your records. At this time you can also verify your contact information. If the information is incorrect, press your back button in the browser and correct the information.
